Boomer Esiason Foundation Scholarship Program
The Boomer Esiason Foundation (“BEF” or “the Foundation”) has created a scholarship fund to assist cystic fibrosis (“CF”) patients pursuing undergraduate and graduate degrees. As therapeutic developments increase the lifespan of CF patients, the need for a scholarship program for these students has emerged.
Overview of the Scholarship Program
Scholarships will be awarded quarterly (March, June, September, December) on the basis of demonstrated financial need and academic accomplishment. BEF’s target is to grant 10 to 15 scholarships each year to CF patients pursuing academic goals. Grants are made directly to the academic institution to assist in covering the costs of tuition and fees.
The BEF Scholarship Program will offer grants ranging from $500 to $2000. quarterly.
Candidate Evaluation
The nominee’s scholastic ability, character, leadership potential, service to the community, and need for financial assistance, will be considered by the BEF Scholarhip Program committee when selecting recipients.
Applicants are required to provide ALL the following documents in order to be considered:
- Application.
- Recent photo.
- Letter from physician (on letterhead) confirming CF diagnosis and therapy routine.
- Letter from social worker (on letterhead) confirming need for financial aid.
- Most recent W2 form verification for both parents.
- One-page essay stating post graduation goals.
- Letter of acceptance from academic institution.
- Detailed breakdown of tuition costs from academic institution.
- Transcript (high school, college, or graduate school).
- Phone interview: will be granted to top 10 finalists per quarter.
Scholarship recipients must be willing to participate in BEF’s CF Ambassador Program. CF Ambassadors will speak once a year at a designated CF event and help educate the general public about CF. The Boomer Esiason Foundation will pay for Ambassador’s travel and lodging expenses.

AWARD
Gunnar H. Esiason Endowment Scholarship Fund at Hofstra University
CRITERIA
A student who has cystic fibrosis, or who has a family member stricken
with the disease, or a full-time undergraduate student whose service and
commitment to the prevention and cure of cystic fibrosis is exemplary.
SELECTION COMMITTEE
Boomer Esiason, Director of Student Activities, Associate Director of
Student Activities, Vice President for Public Relations, Campus Life
Representative, President of Phi Sigma Sorority or student leader chosen
by the sorority if president is eligible for the scholarship.
SELECTION PROCESS
· Candidate will complete an application
· The committee will choose finalists who will be interviewed
thereafter.
· The award recipient will be chosen by a majority vote.

Also available for people with CF is the CREON Family Scholarship Program . Applicants for these awards may be in high school, college, or graduate school. Awards are based on academic achievement, financial need, leadership qualities and ability to serve as a role model to others who have CF. Applications are available at CF care centers nationwide from February through June. You also may visit the Family Scholarship Web page for more information. Each eligible student will receive $2,000 per year for up to four years. Applications are due in June and awards are made in August.
